The 21-Day Launch Guarantee

Website, catalog, pricing and subscriptions by day 10. Everything built and ready to take patients by day 21. Our fractional ops team runs pharmacy, doctors network and customer support through week 16.

If we miss day 21

We keep working at no extra cost until you are ready. Not ready by day 42: cancel and get the whole $30,000 back. Conditions in Q5.

The marketing guarantee

With the fractional CMO, if your cost per new patient is not at or below your Game Plan target for 30 days in a row by week 12, we keep working one extra month at no fee. Conditions in Q5.

How you pay

The build fee is paid in full at signing, plus $5,000 a month for the ops team for the first 4 months (first month at kickoff). There is no other payment plan. Refunds come only from the guarantees: the full $30,000 if you are not ready by day 42, and the CMO fee back if the agreement ends before your first campaigns go live.

Third-party costs

Pharmacy, prescribers, processing and ad budget are billed to you directly. No markup, no cut. We pay your platform fee for the 16 weeks.

After week 16

Continue on a 12-month partnership ($5,000 a month or 10% of profit, whichever is greater, plus 10% of an exit) and the ops team and platform stay included. Or go on your own: platform $2,500 a month, and our ops team $5,000 a month if you keep it.

Ownership

Brand, domain, accounts, patients and data are yours from day one.

Your agreement · updates with your plan

Generated from the pricing and scope you select in "Your plan". Change an add-on and this agreement and its link update. Signing opens 30 minutes after your call; if you are ready sooner, use "Request to sign now" at the bottom of the agreement and we will approve it.

vs
Platform or operating team?

Not a platform. Not a course. An operating team.

We are not the cheapest way into telehealth, and we are not here to teach you how to start one. We set up, scale and operate an entire new revenue stream inside your portfolio of businesses, so it builds enterprise value you can one day sell.

It is not apples to apples. It is apples and oranges. You can absolutely start on one of these platforms and do it all yourself, and plenty of people do. How the table works

The assumptions, and the math.

Each white cell is an assumption you can change. Here is what each one means, the range we see across brands, and why we start where we do.

InputWhat it meansTypical rangeDefaultWhy
Monthly ad spendWhat you pay Meta, Google and other networks in month 1. Paid by you directly to the platforms.$10K to $100K$30,000Below $10K a month most brands cannot get enough data to optimize. $30K is a common first-quarter budget.
Ad spend increase each monthHow much the budget grows month over month, compounding. Month 2 spend = month 1 × (1 + increase).0% to 50%30%Brands scale spend while cost per patient holds. In practice growth slows as budgets get large, and cost per patient tends to rise with scale.
Ad cost to get one new patientAd spend divided by the number of people who paid for their first month. Not cost per click or per lead: cost per paying patient.$200 to $500$300New brands start near the top of the range. Strong creative, creators and a call center working abandoned carts pull it down.
What a patient pays youMonthly program price, including medication and the clinician visit.$199 to $449$299Compounded weight-loss programs cluster at $249 to $399. Wellness and hormone programs run lower.
MedicationPharmacy cost for one month of treatment, at our network pricing.$38 to $100$65Tirzepatide runs $50 at dose 1 to $100 at dose 6; $65 is the average over the first four doses. See Pharmacy pricing.
Doctor visitsPrescriber fees for the initial visit and refills, spread across the months a patient stays.$15 to $35$20Initial visits cost more than refills, so the monthly average falls the longer patients stay.
Shipping and suppliesCold-pack shipping, syringes, alcohol swabs and packaging, per monthly order.$10 to $25$15Cold-chain shipping is the main driver.
Card processingCard processing at a flat 2.9% of the price. It updates when you change the price. The platform's monthly fee is a separate row.2.9%$8.67A flat 2.9% on every charge. On a $299 program that is $8.67.
Patients who stay each monthThe share of paying patients who renew each month. The rest cancel.75% to 90%85%85% means a patient stays about 6.7 months on average (1 divided by the 15% who leave).
Existing customer listCustomers you already have, from any product or price point. Matched to a treatment category and marketed by email, SMS, retargeting and phone.3% to 15% convert5,000 · 60% fit · 10%The fastest, cheapest patients you will ever get: no ad cost, and they already trust you. They subsidize the paid channel from month one.
RefundsShare of revenue refunded or charged back.3% to 20%5%A good ops team keeps refunds under 5%; platforms with weak ops teams can run as high as 20%.
Telehealth platformThe platform's monthly fee.Varies by platform$2,500Paid by us for the Full Build's first 4 months, and included if you continue on a partnership. On the Partnership path it is your current platform's cost.
Your own operations teamYour own staff or contractors running the business each month: admin, inventory, bookkeeping, a support lead. $0 while our fractional ops team runs it (4 months on the Full Build, 12 on the Partnership).$0 to $15K$5,000A lean brand runs on one ops hire or part-time contractors. It grows with patient volume.

How each row of the month table is calculated

  1. Ad spend = month 1 spend × (1 + monthly increase) for each month after the first. At 30%, month 2 is $39,000 and month 3 is $50,700.
  2. New patients = that month's ad spend ÷ ad cost per new patient. Month 1 at $30,000 and $300 is 100.
  3. From your customer list (if you have one) = customers × share who fit a category × share who convert, spread across the launch campaign months, plus new list customers each month at the same rates. These patients cost $0 in ads and are added to new patients. 5,000 × 60% × 10% over 3 months is 100 extra patients a month at launch.
  4. Paying patients = last month's paying patients × the share who stay + this month's new patients. Month 2 at 85% and 30% growth: 100 × 0.85 + 130 = 215.
  5. Revenue = paying patients × what a patient pays you.
  6. Patient costs = paying patients × your total cost per patient (medication + doctor + shipping + fees).
  7. Profit this month = revenue − refunds − patient costs − ad spend − your own operations team − telehealth platform (− the call center's per-sale cost, when it is on; on the Partnership, − the $5,000 fee in months 1-12 and − 10% of a profitable month's profit).
  8. Running total starts at minus your plan's upfront total and adds each month's profit. The month it turns positive is your payback.

Force multiplier assumptions

InputWhat it meansTypical rangeDefault
Extra patients from called abandoned cartsPeople who start intake or checkout and leave, then convert after a call or text from the call center.+5% to +15%+10%
Patients who ascend to a higher planPaying patients moved to a higher dose, a bundle or an added treatment.10% to 30%20%
Extra profit per ascended patientAdded price minus added cost, each month.$50 to $150$100
Call center cost per salePerformance only: no monthly fee. Paid per patient it converts from an abandoned cart or upgrades to a higher plan.$50 to $100$75

With the multiplier on, new patients rise by the recovery percentage on the same ad spend, and each month's revenue adds (share who ascend × extra profit per ascended patient) for every paying patient. The call center is paid per sale it closes: (recovered patients + newly upgraded patients) × its cost per sale; with no sales it costs nothing. Not included anywhere beyond the operations line: owner salaries, office, legal and accounting, or taxes. Refunds and chargebacks have their own row. Ranges are what we see across brands, not a promise of yours.

Why HRT cream, why now

The label changed

In November 2025 the FDA announced it would remove the "black box" warning from estrogen hormone therapy, with new labeling in early 2026. Women who were told no for twenty years are being told yes.

Demand is far ahead of supply

Hormone therapy use among US women 40 to 64 fell from about 27% in 1999 to under 5% by 2020. Around 1.3 million women reach menopause every year, and most with symptoms go untreated.

She stays for years

Weight loss has a finish line; menopause care does not. Retention is driven by service, not by reaching a goal, and one 90-day jar replaces monthly cold-chain shipping.

Typical candidateSymptomatic, under 60, within 10 years of menopause, has a uterus (the progesterone protects it). Prescribed on symptoms and history; usually no labs to start.
Declined or referred outHistory of breast or uterine cancer, blood clots, stroke or heart attack, unexplained bleeding, active liver disease, pregnancy. The clinician decides; your funnel should not target or promise these patients.

Compounded medications are not FDA-approved finished drug products and are prescribed only after clinician review. Marketing uses symptom-relief language only, with no cure or prevention claims. Figures are illustrative ranges, not a promise of results. Usage data: JAMA Health Forum analysis of NHANES, 1999 to 2020; FDA announcement, November 10, 2025.

Q3
How does it work

Everything done in 21 days. Our ops team for 4 months.

Three phases. We do all the work in the first three weeks: your website, catalog and pricing by day 10, everything ready to take patients by day 21. For the full 16 weeks (4 months) our fractional ops team runs your pharmacy, doctors network and customer support, and advises you as you grow. Each one lists what we deliver and the few things we need from you, so nothing is a surprise. Already live? On the 12-Month Partnership there is nothing to build, and the same ops team runs your brand for 12 months.

Your ops team, in detail

What our ops team does for every patient. And for how many.

Your included capacity

500active patients a month
0Heads-up at 400500

Included for your whole ops term: 16 weeks on the Full Build, 12 months on the Partnership. An active patient is anyone with a subscription, order or refill that month. After the Full Build's 16 weeks, the ops team stays included if you continue on a partnership, or you can keep it for $5,000 a month.

Above 500: $1,000 a month for each extra block of 100 active patients, or one flat monthly retainer if you prefer a fixed number. We tell you in writing when you reach 400, so it is never a surprise. It pays for our team's time and is never a cut of a prescription.

For every patient

  • Order, shipping and delivery problems solved with the pharmacy, including reshipping lost, late or warm packages
  • Refills and renewals kept on schedule with the prescriber
  • Billing, failed payments, pauses and cancellations handled
  • Refund requests handled under your policy, with the reason logged so we fix the cause
  • Questions answered by phone, email and text the same business day
  • Anything clinical routed to the prescriber the same day. Our team never gives medical advice

By prescription

GLP-1 injectables (semaglutide, tirzepatide)Dose-step refills timed with the prescriber, cold-chain and warm-vial replacements, needles and supplies, side-effect reports routed the same day.
Hormones (HRT cream, TRT)Lab orders and results routed to the prescriber, refills tied to renewals, 90-day jar timing.
Peptides and other injectables (sermorelin, NAD+, glutathione)The pharmacy's instruction sheets and supplies, refill cadence, dosing questions sent to the prescriber.
Oral and topical (enclomiphene, oral hormones)Refill cadence, shipping and plan changes.

Why the ops team matters: refunds

Weak ops team
up to 20%
Good ops team
under 5%

Slow support, lost shipments and missed refills turn into refunds and chargebacks. Platforms with weak ops teams can see refund rates as high as 20%. A good ops team keeps them under 5%. On $100,000 a month in sales, that gap is $15,000 a month. Change the refund rate in Q2 to see it in your own numbers.

Why we never hand patients to a VA

Broken or warm vials, missed shipments, side effects: patient care needs people who have done it for years. The least experienced person on our care team has five years in telehealth.

Why the long intake stays

Checkout-first funnels convert faster on paper but bring higher refunds and chargebacks and do not hold up in an audit. A full intake protects your processing and your license partners.

Justin walks through analytics from campaigns his team runs.

The podcast: Blake and Mark on telehealth (1h 17m)

A long conversation about the industry and how we think. Background, not homework.

Every question, answered

Who owns the brand and the patients?

You do. Brand, domain, accounts, patient list and data are yours from day one.

Can I keep my current store or practice?

Yes. Your telehealth brand runs alongside what you have today.

How fast can I take my first patient?

You are ready to take patients by day 21, under our 21-Day Launch Guarantee. We run pharmacy, prescriber and processing setup in parallel so nothing waits on anything else. LegitScript, if you add it, can finish after launch.

Do I need LegitScript?

To advertise prescription telehealth on most ad platforms and keep processing stable, yes. Add it to your plan for $4,000. We handle the payment to LegitScript.

How much ad budget do I need?

Plan on at least a month of advertising on top of the build. We size it with you in your Game Plan.

What do prescribers cost?

A per-visit fee for initial and refill visits, quoted with your pharmacy pricing so you see full cost per patient.

How does the 12-month partnership work?

Two cases. If your brand is already live, the 12-Month Partnership is a path of its own: nothing to build, the greater of $5,000 a month or 10% of monthly net profit for 12 months ($5,000 at signing), plus 10% of an exit during the partnership. If you start with the Full Build, you can continue after week 16 on a partnership for $5,000 a month or 10% of profit, whichever is greater, plus 10% of an exit, and the ops team and platform stay included.

Why do I need you if the platform already has pharmacies?

Pharmacy pricing moves fast; GLP-1 pricing alone has dropped sharply in six months. Platforms earn their margin on the prescription, so they rarely pass savings on. We negotiate directly, rotate three to five pharmacies for redundancy and reprice every quarter, and when we bring our own negotiated rate to a platform, the difference comes back to you.

Can I sell non-prescription products too?

Yes. Supplements, coaching and info products can be bundled into the same checkout. We plan who makes and ships them during your build.

Can I bring my partner or spouse to the call?

Please do. Share this page with them first.

Q5
Our guarantee

Our guarantee to you.

Two guarantees: one for the build, one for the marketing. Each comes with the conditions that make it hold, written into your agreement.

1The build guarantee · 21-Day Launch GuaranteeApplies to the Full Build path

Your brand is built and ready to take patients by day 21 of kickoff, guaranteed. If we are late for a reason on our side, we keep working at no extra cost until you are ready. If you are not ready by day 42, you can cancel and we refund the full $30,000 within ten business days. Either way you keep everything we built, and you own your brand, domain, accounts, patients and data from day one.

The guarantee holds when:

  • The clock starts at kickoff, once the agreement is signed and paid.
  • You send what we ask for (documents, logins, brand and pricing approvals) within 2 business days. Each day late moves the date by a day.
  • Your treatments and brand are locked by day 3. Changes after that move the work they affect.
  • Time a third party takes to approve something we have fully submitted does not count: merchant processing underwriting, LegitScript, pharmacy or prescriber credentialing, platform approval.
  • You give one round of consolidated feedback on the day 10 website within 2 business days.
  • "Ready to take patients" means your site is live on your domain, intake and checkout work, prescriber review and pharmacy ordering are connected, and a test order has gone through end to end. It does not depend on ad accounts or your first sale.

The guarantee covers the Full Build fee. Add-ons follow their own terms. Every word of this is in your agreement under Q1, Custom Sample Agreement.

2The marketing guarantee · one extra month if you miss your numberApplies with the Fractional CMO (Marketing Team) add-on

Ad results depend on things no one fully controls: the platforms, your market and your offer. So rather than promise a number we can't control, we commit to the part we do control: the work. On day 10, your Launch Game Plan sets a target cost per new paying patient from your margins. If your fractional CMO term ends and you have not hit that target, our marketing team keeps working one extra month at no fee. The extension then ends, whether or not the target is reached.

Hitting the target means your blended cost per new paying patient (all ad spend divided by all new paying patients) is at or below the target for 30 days in a row, measured from your own ad and platform accounts.

The guarantee holds when:

  • You fund at least the agreed monthly ad budget, on time, every month. Your ad budget is always yours; the guarantee covers our team's time, not media.
  • Your offer, pricing and treatments stay as set in the Game Plan, unless we change them together in writing.
  • You approve new ads, pages and messages within 2 business days.
  • Your ad accounts, payment processing and, where required, LegitScript stay in good standing. A ban, processor hold or platform policy change we did not cause pauses the clock until it is resolved.
  • Intake, checkout and lead follow-up stay live, with leads worked the same day.

We do not guarantee patients, revenue, profit or a return on ad spend. We guarantee the work: 12 weeks, plus one extra month at no fee if you miss the target. Written into section 3A of your agreement.

$
Your all-in cost

Every cost line, by vertical, against what patients pay.

What a patient actually costs you each month on our network: medication, prescriber, shipping and processing, with nothing left out. A healthy program keeps the all-in cost at or under about half of the patient price, which leaves the other half for marketing, staff, operations and profit.

The cost lines

Medication: our network pharmacy price; GLP-1s are priced by dose. Prescriber: $18 to $25 per visit, typically $20 async; a six-month authorization is one $20 visit, about $3.33 a month. Shipping: $19.99 two-day for cold-pack injectables, $12 ground for oral and topical, and one shipment covers a 3-month supply. Processing: a flat 2.9%. Platform: no per-prescription fee; covered for your first 16 weeks, then a flat monthly fee.

What the numbers say

Semaglutide at $199: about $90 all-in. Tirzepatide: about $114 on average, and about $149 at the top dose, still half of a $299 price; price higher doses higher or bill quarterly. On low-cost medications, the prescriber and shipping are the cost: hair loss billed monthly fails at $49, while a 3-month plan costs $18 a month. Testosterone is a controlled substance and is quoted separately.

What this covers

Pharmacy cost per unit as listed (per vial, kit or tube; per capsule or tablet where shown). Dose 1 is the starting dose; higher doses follow the titration schedule. Pharmacy cost for the medication only. Prescriber visits, shipping and platform fees are estimated in the Q2 numbers and confirmed on your call, so you see the full cost per patient before you sign.

How the price holds

We order across three to five partner pharmacies and reprice every quarter. When a price drops, your price drops with it. Quote valid 30 days, confirmed at onboarding.
